ExtraDoctors Terms of Service

§1. General provisions
1. These Terms and Conditions define the rules for using the ExtraDoctors website, available at: www.extradoctors.pl .

2. The service provider is: Katarzyna Yachenko, a specialist in individual medical practice, registered at: 14 Sluzova St., Boleslavets, 59-700, TIN: 6121884308, REGISTRATION NUMBER: 527367221.
3. The website provides remote medical services (teleconsultations) in accordance with the current Polish legislation.

4. The Terms and Conditions are available on the website and can be downloaded by the user.

§2. Definitions
1. The website is the ExtraDoctors online platform.

2. A patient is a person who uses medical services.

3. A doctor is a person licensed to practice medicine in Poland. 4. Teleconsultation – medical services provided remotely (by phone or online).

5. Service – any service offered on the website.

§3. Scope of services
1. The website allows you to:
• conduct online medical consultations,
• write electronic prescriptions,
• Issue electronic sick leave (e-ZLA),
• provide referrals to specialists (if necessary for medical reasons).

2. Services do not replace a personal visit in cases requiring a physical examination.

3. In case of emergency, call the emergency number 112.

§4. Terms of Use
1. To use the website, you must:
• device with internet access,
• the ability to communicate by phone or online.

2. The patient agrees to provide truthful and complete information about his health.

3. The doctor has the right to refuse to provide the service in the following cases::
• insufficient medical data,
• suspected illness requiring an in-person examination,
• Providing false information.

§5. Registration and payment
1. Patient registration on the website is required to use the service.

2. Services are paid in accordance with the current price list available on the website.

3. Payment is made online through the PayU payment system.

4. Payment is required to provide the service.

§6. Responsibility
1. The doctor is responsible for the services provided in accordance with applicable law.

2. The Service is not responsible for:
• consequences of providing false information by the patient,
• technical problems beyond the control of the service.

3. The patient acknowledges the limitations of teleconsultation resulting from the lack of physical examination.

§7. Complaints
1. Complaints should be sent to the following email address: info@extradoctors.pl 2. Complaints should contain the patient's details and a description of the problem.

3. Complaints will be reviewed within 14 days.

§8. Protection of personal data (GDPR)
1. The controller of personal data is: Katarzyna Yachenko, a specialist in individual medical practice, registered office at 14 Sluzova St., Boleslavets, 59-700, tax identification number (NIP): 6121884308, number in the National Register of Enterprises (REGON): 527367221.
2. Personal data is processed for the purpose of providing medical services and maintaining medical records.

3. Patients have the right to access, correct and restrict the processing of their data.

4. Detailed information is provided in the Privacy Policy.

§9. The right to withdraw from the contract
1. The patient agrees to start providing the service within 14 days of the conclusion of the contract.

2. The right to withdraw from the contract is not granted after the provision of the service.

§10. Final provisions
1. These Terms and Conditions are governed by Polish law.

2. In matters not regulated by these Terms, the applicable law applies. 3. These Terms and Conditions shall enter into force from the date of publication on the website.
